Forms
Every way people reach you in one place: enquiry forms, booking pages and questionnaires, with embedding and results.

Forms gathers every form a person can reach you through. You do not build them all here. It is the one place to see them, share them, embed them and read the results.
The four kinds
- Lead capture: your enquiry form, and any routing forms. See Leads and Routing forms.
- Scheduling: your free event types. See Event types and booking links.
- Instant booking: event types that take payment or carry an agreement.
- Questionnaire: a form whose answers land on one of your boards as a new item. See Boards.
Each card shows whether the form is Live, Off or Needs setup.
Make a form
- Choose New form.
- Pick what it is for. Each choice takes you to the right builder.
A questionnaire fills in a board, so you need a board first. If you do not have one, Forge offers to make one.
A form's tabs
Open a form to find:
- Build, to edit its questions.
- Design.
- Embed, to share it or put it on your website.
- Results, the responses so far.
- Analytics.
Put a form on your website
Open the form's Embed tab. There are three ways:
- Inline sits in your page and grows to fit each step.
- Popup adds a button that opens the form over your page.
- Full page is a link to the form on its own page. There is nothing to paste.
Copy the code Forge gives you and paste it into your site. The tab has steps for WordPress, Squarespace, Showit, Wix, Webflow, Framer, Shopify and anywhere else. If your site builder does not allow scripts, use the iframe version shown there.
Booking pages need a booking link set in Settings → Profile before they can be embedded.
Analytics
Analytics shows Views, Starts, Submissions and Completion rate, over the last 7, 30 or 90 days.
These are counted without cookies or anything that identifies a visitor. That means someone who reloads the page counts twice.
After someone submits
Your enquiry form and questionnaires can show your own confirmation message, or send the person on to a web address of yours.
